Fatigue-severe, unexplained, and worsening for the last 6+ years. I can't drive safely, can only do about 50% of job responsibilities.
ABOUT THIS CASE FILE
The impact of fatigue is more severe mentally than physically. The degree of brain fog is high, my memory, ability to make decisions, hold attention and conversations is terrible. It seems I cannot force my brain to function like I can my body. Fatigue has caused me to mostly give up driving, I'm only 35 years old and barely hanging onto a shell of my job, unable to safely do most of what I enjoy in life. While difficult, I can force myself into physical activity (I try to several times every week), walk, jog, bike, etc. I feel pretty healthy/normal while my heart rate is elevated, only noticeable impact of fatigue during activity is increased risk (poor decision making and clumsiness). My physical performance/endurance seems unaffected. Post activity, as soon as my heart rate normalizes I return to a level of fatigue same as prior to activity. Chronic Fatigue Syndrome suggests extreme exhaustion after mental/physical exercise, this is not the case for me. I have no such Difficulty sleeping (especially after stopping mirtazapine for sleep aid) but no improvement after a good night, treating apnea had no effect. Crohn's disease in remission with Stelara. Still have diarrhea but no cramping bleeding. One doc suggested depression, no antidepressants helped. Adderall helped but lost efficacy after a while. I have seen Nuero, Nephro, Endo, Infectious Disease-they've found nothing abnormal. Nutritionist said not enough calories (despite weight gain, and since remedied-following prescribed died); Cardio EKG normal, Echo scheduled. Rheumatolgy refuses to see me. Psychiatrist saw no depression but trying to get appointment with psychology anyways. My primary care doctor is treating me for borderline hypothryroidism but I have noticed no improvement on levothyroxine. My resting heart rate is quite irregular (high 40's to low 70's). I've had my house checked for mold, radon, and general air quality-no issues. City water but recently installed reverse osmosis. I live with my wife and dog, wife has had complicated thyroid issues that predate our relationship (no new related symptoms) and dog seems healthy-environmental seems unlikely. I've tried a lot of vitamins and supplements (from Ginseng to Glycine, CBD to caffeine pills) and prescriptions (Modafinil to amphetamines). Have had many tests: Celiac biopsy, head MRI, abdominal ultrasound, chest x-ray, testosterone, regular CBC & metabolic panel. Many other tests and meds that I can't remember at the moment. Despite being willing to try just about anything, I'm about out of resources and ideas. I've been staying more mainstream to utilize my good health insurance but am willing to branch out wherever it makes sense. Chronic fatigue syndrome is confusing, as it sort of doesn't exist. That said, my research on the topic says I don't have enough of the listed symptoms for a match. Please ask me any questions, I'm pretty open/honest. Thank you in advance!
