ABOUT THIS CASE FILE
Medical Conditions: Alpha 1 antitrypsin deficiency, 9 years post bilateral lung transplant, osteoporosis, GERD, Small fiber neuropathy/ganglionopathy.
Severe pain basically from head to toe. Increases as day goes on. This is a list, with a few changes, of information given to Mayo Clinic. Where pain is and what it feels like, at its worst, top to bottom:
Head: pain through entire head, worst in forehead, across the bottom of the skull and behind ears. Hair even hurts. Feels like brain will explode. Intensifies when I lay down.
Face: throughout face, up cheeks (burning and tingling) feels swollen. Pain inside jaw at all times. Burning (like severe road rash in jaw, tongue and back of mouth near throat), gnawing, throbbing will go to all teeth up and down. Two teeth closest to the removed tooth area most severe pain, feels and tastes electrical. Foul breath and very dry mouth. Also feels swollen and sometimes seems hard to swallow, food goes part way down, and then comes back up. Pain and burning in eyes, feel like they could just “pop” out of the socket, blurred vision, very dry eyes. Feels like face is swollen and stretching the eye downward. Ear pain, feels like they are “bleeding”, ringing in ears. Can place pressure on jaw with small amount of pain, but a blanket being pulled across my jaw causes lots of pain. Unable to chew much of anything due to pain. Eating intensifies the burning and pain in the mouth. Swelling under jaw (lymph node area) gums where teeth are gone turn white
Neck: Mainly back and sides of neck can trace it from behind ear, down neck, down back. Burning and aching, tightness. Swelling right side
Shoulders: Pain with lifting, lying on shoulder. Sitting in chair with back against the chair.
Back: Mostly halfway down back. Many “pressure” points of pain, especially right side just to the right of my spine. Feels like hot poker is stabbed halfway down my spine, mainly on right side.
Chest: Titanium rods, wearing a bra, incision scar area, breasts
Hands and arms: Numbness, tingling and pins and needles in hands starting to go all the way up arm to shoulder. Pain intensified in thumbs. Feel unable to grasp and hold, like dexterity is lost. Arms feel “Heavy” at times. With aching deep inside. Hands usually very cold.
Legs and feet: Tingling, burning in feet and calves. Top of right foot near toes area of more pain than the rest of the foot. Can press spots on the feet and send pain up the body. Feet usually feel very cold goes up about 4 inches over ankle.
Skin: Itching, especially in arms and legs. Red rash on face near both sides of nose, flushing when face feels really hot
Mental: Terrible brain "fog", depression (which would not exist if pain could be controlled), sadness (due to loss of ability to live the life I am used to) anger (for all this has taken from me) frustration (no one seems to be able to tell me what is causing all this pain)
General: Very small tolerance window for hot and cold. Frequent chills then feel very hot.
Things that increase pain: Normal day to day activity, just increases as day goes on, by mid afternoon or evening many days a week it is so severe I end up in bed crying and writhing in pain. Stress will take it off the charts. No position is comfortable anymore, I end up sitting, standing, walking, lying, just keep trying to change position to help. Changes in weather.
How it has adversely affected my life.
I was a RN and am unable to work due to the pain and severe brain fog. My husband and I love to travel and I have had to cancel trips. Less enjoyment of social activity. I am learning to bear the pain and am trying my best to go on with my life. But I would really love to have a diagnosis and possible treatment or decreasing of pain.
Practitioners seen and testing done:
Dentists, oral surgeons, endodontist, neurologists, with no actual diagnosis that explains the extreme pain I am experiencing and the continuous progression. They have run dozens of blood tests, a facial CT, 2 brain MRI's, 2 EMG's, skin puncture biopsy, nerve biopsy.
Only medication for this problem is gabapentin, with very minimal relief. Tried cymbalta and nontryptiline with adverse side effects. Tried tramadol and muscle relaxer with minimal relief. I take other medications and vitamins for transplant.
Treatments or procedures:
Upper cervical and regular chiropractor, acupuncture, massage.
