ABOUT THIS CASE FILE
I’m a 62 year old female. About a year ago this all started out of the blue. I started only being able to eat small meals. I went from eating a normal diet to not being able to eat more than a couple hundred calories a day. Now I am down to eating an egg a day with one other small snack and that is all my body can handle. I can’t even handle many liquids. I drink about two cups of water a day and that’s all I can handle. Once I consume the egg or the small snack I get full hours later. Full to the point where I can’t wear anything on my stomach like pants or my seat belt. I have chronic bloating. But my biggest symptom is chronic belching. What sets off the beaching is me sitting or bending my body. This generally only occurs at night hours afternoon I have eaten my egg and snack. Once I have sat I start the belching it won’t stop. I need to stand and walk to get relief. And this will continue for hours on end. If I try to add anything else into my diet In a day I get very sick to my stomach. I develop upper right quadrant pain after a “meal”. The bloat is so bad I can’t even walk. The benching gets worse. I try to taken pepto, omperazole, baking soda, gas ex and nothing relieves my symptoms until I go to bed. The pain will be so bad that I have to take ambien to go to bed. I also suffer from chronic constipation. I am on two laxatives a day and still cannot go more than twice a month. As a result I have lost 40 pounds. Drinking more than about two cups of water a day exacerbates these symptoms. I have had many tests. I have an accelerate gastric emptying into my stomach at the two hour mark. So gastroparesis was ruled out. I had a sitz markers test and I did not eliminate any of them. But no diagnosis. I have been treated for pancreatic insufficiency with Creon and it did not impact me in a positive way. I’ve had negative breath tests and also negative SIBO. Negative chest CT. Negative enterography. Negative colonoscopy and endoscopy with only polyps in my stomach. When my symptoms were first starting I also developed iron deficiency anemia. I had two iron transfusions and that cured that part of this disorder. This impacts my life because of how much weight and muscle I’ve lost. I have no life at this point. I cannot go out to eat with others, cannot enjoy a meal, I struggle with my constipation every single day.
