- CASE FILE
Chest pain created by stomach filling with air and expanding in chest.
ABOUT THIS CASE FILE
Severe chest pain that appears to be related to stomach swelling with air and expanding until it pushes on lungs, chest muscles and ribcage. Initially presented as vomiting anything ingested - not just food but also liquid including water - which led to severe malnutrition causing changes in skin texture and hair loss. Pain centered in middle of rib cage and can radiate to back if no steps are taken to reduce the pressure. Air accumulation starts slowly once upright (occurs when sitting, advances more quickly when standing). Increases drastically with movement/exercise - particularly with upward movement of legs into the hip joints. Relief occurs as soon as laying down/prone and even more dramatically when bending spine backwards (as in a back bend position). Non-erosive reflux with neutral PH. Also present are a paralyzed left vocal cord (could be from being intubated) , deviated septum and significant lump on thyroid. Could they be a factor? Testing has been completed for hiatal hernia, gastroparesis, GERD, eosinophilic esophagitis, gluten allergies and more. MRIs, CTs, gastric emptying, endoscopies, colonoscopies and ultrasounds have been completed. Would a standing MRI identify something else? Solutions tried include antacids, reglan, sucralfate, acupuncture (5 treatments), chiropractic, physical therapy and more. The only thing helpful long-term has been baclofen which relaxed stomach enough to keep down liquid and foods ingested. This allowed for improvement but not a cure. Lifestyle has been completely altered to reduce physical activity and movements/bending into the chest region.
